Tattoo Consent Agreement

• I confirm that I am at least 18 years old, or I have valid parental or legal guardian consent if I am under 18.
• I confirm that I am not pregnant, nursing, or suffering from any condition that may affect the healing process of a tattoo.
• I confirm that I do not have any medical or skin conditions that could interfere with the tattooing process (such as keloid scarring, psoriasis, or open wounds), or I have disclosed such conditions to the artist.
• I confirm that I do not have any allergies to latex, gloves, soaps, inks, adhesives, or medications, or I have informed the artist of any known allergies.
• I confirm that I am not under the influence of alcohol or drugs at the time of the tattoo session.
• I understand that getting a tattoo involves permanent changes to my body and that there are no guarantees for restoring the skin to its original condition.
• I acknowledge and accept the risks involved with tattooing, including infection, allergic reactions, scarring, and other potential complications.
• I confirm that I have received aftercare instructions and I agree to follow them strictly throughout the healing period.
• I understand that I may feel dizzy, faint, or lightheaded during or after the procedure and will notify the artist if I experience any symptoms.
• I confirm that all equipment used is sterilized and disposable, and proper hygiene protocols are followed.
• I understand that all tattoo services and related sales (such as inks or aftercare products) are final and non-refundable.
• I give permission to the studio to take photos of my tattoo for promotional use (such as website, social media, and portfolios), unless I explicitly request otherwise.
• I release the tattoo artist and the studio from all liability related to this procedure and acknowledge that I am getting the tattoo voluntarily and at my own risk.
